Medical Clearance Forms 2023-24

On this page, you will find information about the items that need to be completed prior to participation in Endicott College Athletics this upcoming year. The information you provide in this paperwork is essential to providing the best care for all student-athletes.

This paperwork is MANDATORY for all varsity and club student-athletes, as well as all student coaches and student managers. Students will not be eligible to participate in team activities until all aspects of the forms are complete and have been reviewed by the Athletic Training staff.

The deadline for information/paperwork is as follows:

ALL STUDENT-ATHLETES (FALL/WINTER/SPRING/CLUB TEAMS) – DEADLINE AUGUST 1, 2023

All paperwork submitted after the specified deadline is subject to a 48-hour processing period. During this period, student-athletes with tardy or incomplete paperwork will not be allowed to participate in their sport.
