Transportation Release Form

This form must be completed and meet the following guidelines for your request to be granted.

FORM GUIDELINES

weekday game (Monday-Friday) requests must be completed and received 24 hours prior to the scheduled event
weekend game (Saturday/Sunday) requests must be completed no later than 5 PM on Friday
requests made for games on Holidays that fall on a Monday must be completed no later than 5 PM on Friday
the student-athlete named on the form will only be released to the responsible parent or guardian listed on the transportation release form.

Failure to submit this form at the appropriate deadline(s) will automatically void authorization of departure.

The form will be on file with the Athletics Department, and a copy will be given to the student-athlete's head coach prior to the team's departure from campus.
